The first few tries going against the Lich's, I lost multiple party members. My usual party goes as such, Shiela (Main) Mage, which is my avatar,Minsc, Sir Anomen, Yoshimo (Imoen replaces), Jaheira, and Viconia. Though it is a weird assortment of characters, I managed to get to the end of the game with these people. Possible Spoiler unless your tired of getting killed by these things. This may only work on the lich's you face on the Kangaxx Quest.: Anyways, you'll need Yoshimo for this. If he can, have him set traps around where the lich will appear, which is near the walkway up to the stone casket. Have your main fighter characters (Minsc, Anomen) on each side of the walkway. Cast every protection spell you know, especially protection from evil 10', because the lich (if he survives) casts time stop, and usually gates in a creature, along with other instant death spells. Have Yoshimo open the casket. Once this happens, have invisiblity purge and breach at the ready. When the lich appears, the traps will do a great deal of damage. If he survives, he'll tend to go invisible at times, which would cancel breach. So cast invisibility purge and then breach (if you survive his first barage of death spells). If breach works, then the fighters will finish it off.
Dont know if it'll work on the other Lich's, but this is a easy way to get rid of those pesky ones on that one quest.

I do almost the same... with a pretty different party.
I'm currently playing with :
- Fighter dual-classed to a Druid
- Keldorn (Instant-Dispel at 2x Level do WONDERS against Liches!)
- Mazzy (Used as a Tank. Currently using Shields + Short Swords, but will dual-wield once I'll have mastered Short Swords.)
Keto (Originaly a bard, but I modified her to be a Skald instead. Support Spellcaster, use ranged weapons and boosting song for combat)
- Jan Jansen (Great Thief, Good Mage!)
- Kelsey (Excellent spellcaster, romancing my PC)
Here is what I've done for the last 2 liches :
- Place PC, Keldorn and Mazzy next to the spawn point. Have Keldorn equiped with Daystar. Have a summon nearby to attract the 1st spell (death spell). Kelsey and Jan out of the Liche's line of sight (after buffings : Death Ward, Chaotic Commands, Remove Fear, etc.). Keto is near the sarcophagus. Have Keto open it, pause, start casting Sunray with Keldorn, have Mazzy attack the Lich, Jan and Kelsey ready for *Detect Illusions* (Thief skill, really useful!) and Breach.
I killed the last lich in 1 round with this Tactic. Daystar's Sunray usually bring the Lich to "Near Dead" or "Badly Injured"... then just hack at it with Keldorn, Mazzy (Mazzy's Simulacrum if you have Vhailor's Helm) and PC, and wait for it to die... breach if necessary, glup potions when needed.

Pre-setting traps is basically a little cheesy (once it comes to spike traps, you can nail anything including Demogorgon instantly with this method). A more proper way to use traps set in advance is to set up traps, then lure the enemy near the traps and push him into them with the ring of the ram (copyright by Bruce Lee I think) .
With Daystar, Helm of Briliance and Helm of Vhailor you can fire three sunrays, one with Daystar by simmie and two by yourself with Daystar and Helm of Brillance.
If the lich summons a Pit Fiend, he's likely dead, because most liches "forget" the Prot from Evil part - just let the Pit Fiend rip the lich apart. The one that guards Daystar is pretty much an expert for this type of suicide. With tactics installed, things can get worse though, since a Pit Fiend can gate in another Pit Fiend then.
The best way to deal with liches is usually not to protect oneself too much in advance (a good lich would hammer you with abjurations anyway), summon some highly magic resistent cannon fodder (typically skelly warriors) to keep it busy during his initial times stops and then to turn it visible (Oracle, True Sight), strip it lich of its protections (Ruby Ray, Pierce Shield, Breach) and simply hack it to pieces.

If you want cheese, send a lone paladin weilding Carsomyr against the lich. It takes a little time, but the paladin doesn't get touched.
In terms of trap cheese, set traps, rest, set more traps, rest, set more traps... etc. etc.
It's ridiculously cheesy, but it's fun to watch. The sound is pretty neat too.

i just let viconia + summons absorb the first spells thx to her high resistances few spells actually went through her magic resistance
also she was buffed up pretty nicely
i just casted pets with my casters (party was a bit to the right standing safe at entrance of the room)
then my meelee chars went in after the most spells were done
for some reason the lich decided to cast time stop the moment my meelee chars walk in
i was probably really lucky only one spell managed to hit a party member
minsk was mazed for a few rounds
and non of my chars substained any dmge ^^
my summons were almost all dead though

Protect a party member. A Paladin is good, he has superior savings, and also the "turn undead" wich is a marvelous tool. But dont use it on liches unless you are a helluva leveled up person. Now, keep summons around the lich to make him busy and keep your party far away. Now he will kill the summons but that will make him lose some time, and also, he will lose his own defenses time while this. When the defenses are down, swarm him with your party and pray for a crit hit. Also, the lich always have a second protection trigger. When he use it, make your party go back and keep the paladin near him again... wait... protections run out... whack him with all party.
That's a non cheesy heavy way to do the job.

Here's what's worked for me (no mods) this time through:
Using Keldorn and main char (dual-wielding Kensai):
Buff with chaotic commands, protn evil, remove fear, death ward.
Position Keldorn on the very edge of sight, with true sight activated.
Potions of magic shielding (or was it the other magic protection potion), speed, invulnerability and giant strength.
Open the sarcophagus, and mash that puppy good, in about two rounds. You'll take some damage but hey.
This won't work on Kangaxx!
Uber-cheese:
Protection from Undead scroll (it just stands there and lets you hack it to bits)

My favourite and funniest trick is simply grab a scroll of Magic Protection and cast it on the Lich.
Though leaning away from cheesy tactics I always prepare a group of summons and spread them around and send them about the place so they simply waste the lich's most powerful spells, then use protection from evil, true sight, protection from fear and start smacking him around.
Also a level 15 skeleton summon makes absolute mince meat out of liches.
