yep, it's a bug, and here's the pesticide...
This is a classic bug that patches don't fix. You have to use cheat codes to create the item. Luckily I just had to do this myself. Here's what you do:
From the Start menu, go into the Black Isle folder and select the Configuration program for Icewind Dale II. There's an option (a little list with a box you can check) to enable cheat codes. Do that.
Then, while you're playing the game, press the CTRL and TAB keys together, and a little "console" will pop up at the bottom of the toolbar/action menu/whatever you call the little frame where you direct the action from. You can type in the console. Type the following phrase exactly (including spacing, capitals, etc.):
ctrlaltdelete:CreateItem("61GENMM")
Then hit the Enter key once or twice (until the console closes and you get the game information dialogue box again). The modified mandrake root should appear in your lead character's inventory (or in the inventory of the next character with an open inventory slot).
I've seen some online guides use different spacing between words and a different item code. THEY ARE WRONG! Use that above phrase exactly. I just tested it again to make double-sure.
