Where is the 4th piece? I have found three.
The one on first level of severed hand.
The one you get for killing Kaylessa(is that how you spell it?)
I am not sure where I got the 3rd one from, but I have been evertwhere looking for another. I have looked at walkthroughs, but can't find one with a good description of where they are
