You want to have your enemies having a brainstorming, huh?

Were exactly in the head's hitbox is not of a matter. Just hit the head. Shooting of a bandits ear is usually enough that his head goes with the ear.

It depends on your enemy wether you can decapawhatever or not.
Only humans can lose their head. Undead, Minions and all other creatures can't.
Secondly your (cross)bow must afflict a minimum of damage. This is somewhere between 33% and 50% of the enemies maximum hitpoints, I don't know the exact figures. So you can't get rid of a bandit's head with your oak bow.
Additionally only the maximum hitpoints count, not the current.
Your enemy has seen you and is great at blocking? Forget it. Guards rushing at you won't block your shots but won't lose their head either. They can avoid these kind of criticals somehow. If the guard doesn't see you. Goodbye head ..
It also depends on one final thing. Is your enemy wearing some kind of helmet? (Guards hats are helmets) Your (cross)bow have to have a greater damage output then.
