need catacombs (prison level) spoiler

I've pretty much cleaned out all 4 (?) levels of the catacombs including teleporting to the circular room on the Prison Level. But, haven't been able to find the passage to the Castle, nor the dwarven area (for the gem), nor some of the elves. Obviously I've missed something. What?
