Keening (SPOILER)

I searched this and didn't find a topic on my specififc question.

I got Keening but everytime I equip it, it deals constant damage to me, could there be any reason for this or is it just a glitch?

When I equiped it, it permanently dmagaed my health by like 200 something points.

Maybe you have to have Wraithguard or something.

Yes, you do need to have Wraithguard.
