I've been wondering around in the Forbidden Lands all day. I've been everywhere, from the Tree Totem Village to the Celestial Shrine, to the fire camp and all the gullies around it, and I still haven't found Minas Taw or the Knight. Right now I'm at the spot marked on the map I found here, at the fighter's quest page, there's an empty hut, and a big stick with some skulls on it at the entrance, but no Minas Taw or sir Falen. Here's a screenshot:

I checked out your save and I see that you have a rediculous inventory, such as a half dozen Magic Mirrors.
You're all decked out with a full set of Death Lord armor and your stats are entirely too low because the penalty is high on the character.
Since you obviously used the editor, it strongly looks like you simply added the quest for Minas Taw. It's no wonder he never showed up at the shack ne of Warglaw in the Forbidden Lands. You have to actually get the quest from someone for things to work out.
