Multiple Enchantments

After reading other threads, ive noticed people saying they have put more than one enchantment on a weapon. when i try it, my only item options are unenchanted ones. can some one explain to me how it is possible to enchant an item more than once?

You have to do it all at the same time (ie. pick two effects for an item when you enchant it. You cannot go back and reenchant an item that is already enchanted.
