Mace +2, The Mauler's Arm - Bernard (after you free the slaves)
Sets wielder's Strength to 18

Girdle of Hill Giant Strength - Adventurer's Mart
Sets wearer's Strength to 19

Girdle of Stone Giant Strength - Rakshasa in Demin's house, Suldanessellar
Sets wearer's Strength to 20

Girdle of Frost Giant Strength - Demon Knight, Altar of Demogorgon (Underdark)
Sets wearer's Strength to 21

Gauntlets of Ogre Power - Halfing cannibal, Planar Sphere
Sets wearer's Strength to 18/100

Crom Faeyr - Cromwell's Forge (consumes Girdle of Frost Giant Strength and Gauntlets of Ogre Power in the process)
Sets wielder's Strength to 25

Machine of Lum the Mad - Watcher's Keep
Can raise all stats by 1 point permanently
(This is great for a character who has 18/xx Strength, since going to 19 makes such a difference)

Throne of Bhaal adds a few more items, including a Girdle of Fire Giant Strength (Yaga-Shura's Lair) and Angurvadal +5 (Cespenar's Forge) and possibly an Ioun Stone or some other item I can't remember.

Also, don't forget about potions.

Runehammer +5 (made by Gespenar):can imbue with Fire Giant Strength once a day, among other benefits;
Angurvadal +5 (Gespenar): strength is set to 22

Ioun stones don't provide any STR benefit.

I think the Blackrazor longsword, found towards the end of SoA, has a chance to temporarily raise Strength (among other nasty effects) upon a successful hit.
Why is it that whenever I finally get around to playing a new game for the first time,
I feel like playing Baldur's Gate for the second time...
