How do they really work? I'm working on a multi-class build (in my head) and trying to decide when to take certain classes.
If I start off as a bard and get 7 levels of a medium base attack bonus of +5, and add some class that has a high b.a.b (like fighter) for 5 levels that gives a b.a.b of +5 where is my character at? Do they stack?
I'm guessing he would have a b.a.b of +10/+5 at level 12 - is this correct?

They stack, yes. Page 133 of the manual has a chart that lists the BAB and Save progressions.
The thing to remember though, is if you start in a new class, you start from level 1 on that chart for each class. Meaning if you are a level 3 Fighter/4 Rogue/4 Cleric, you take the numbers for each class from that level in the class off of that chart. Even if say, the Rogue and Cleric both use the same BAB progression. So, you would end up with a BAB of 3+3+3=9, or exactly the same as a 12th level character with a Medium BAB progression.
