Ok my girlfriend is currently playing through the game and she's on Nar Shaadar. She is controlling Mira and has got to the point in the "pit" where she must fight Hanhar and the Kath Hounds... only then the game flashed and now Mira is in the tunnels again... and neither door opens?
The fight never took place and she shouldn't even be in the tunnels as Mira as she's supposed to be in the actual Complex now where she was about to fight.
Now as both doors are locked, she is stuck and there is no way out... this is the hundredth glitch we've found when playing this game via the Xbox 360.
Are there known problems when played on the 360?

Thank God... I finally know I'm not just imagining things.
Yes, I do believe it to be an issue with the Xbox 360. I, too, have encountered frequent problems while playing KotOR II on my 360 console. These included glitches such as the game telling me that no controller was connected (thus disabling my ability to continue in-game) yet I was able to press my Xbox guide button to return to the dashboard. Also, I get the disc read error quite frequently, even though my disc is in perfect condition. The worst of errors though are in two minor glitches that tend to occur quite frequently. If I'm involved in a battle where there are too many people/droids involved, or too many weapons being fired/swung around at once, I get a major FPS-rate drop, causing a lag that sometimes teleports me places just like in your example. The other, which is merely a nuisance, is frequent little problems with sound, like a sudden short static (which interrupts the music of the game for no more than a second), and of course when certain characters talk, like the TSF general and Atris.
I have many theories on the issues, though the fact that I see that you have encountered the same issues, I am forced to reconsider that it is a problem with my own 360/disc. In my opinion, the most likely thing is that the glitches occur because the backward compatible software sometimes has issues with the game's engine, causing errors that can be as minor as sound interruption, all the way to game freeze, sometimes losing your progress if you've forgotten to save in some time. It's an annoyance, at worst, and I have encountered the same problems while playing other games such as Fable, and Halo 2. Hopefully, Microsoft will be made aware of these errors, and possibly fix some of them when they do their next software update.

Well I'm glad it's not just me as I've saved and saved my money for quite sometime and still couldn't really afford my 360 but brought it anyway, yet these problems really scared me senseless, lol.

360 issue
Ya I've been having the same issue, and with KotOR one aswell, for the Malak fight, I managed to fast-forward right past the trigger point and was standing right next to him, had to backtrack to hit the trigger and have him actually talk to me (so I surrounded him with mines first), but I've been having that problem, the sound issue, and the FPS issue, hoowever I haven't had it lock, or have it think my controller isn't there.
