Something is going horribly wrong with the battle in the Duchal Palace...
As soon as Winski Perorate (a mage) shows up, he immediately casts Flame Strike (a 5th level Priest spell) at my main character causing instadeath.
There is no time to target him with arrows, and the vast numbers of summoned skeletons are also unable to interrupt his casting.
The spell causes 8d8 damage (8-64), with a saving throw for 1/2 damage.
My main character (a mage) has 42 hp, and is wearing a ring of fire resistance for 50% fire damage reduction.
By my calculations, she should always survive with a minimum of 10 hp remaining...
Additionally, she has Greater Invisibilty cast on her (meaning she is untargetable by spells, right?), and I've even sent her upstairs before Winski appears - but the result is always the same.
I'm using TuTu with no additional mods.

If you haven't got it, go back to the Iron Throne HQ and talk to Cythandria on the 5th floor (4th floor if you're British).
