I got the modified root and placed it on the correct table (I got a journal entry that proves it). I then pulled the south lever. It gave me the wyvern poison potion things, but left the mandrake root alone. I then pulled the north lever with my evoker that has a base alchemy skill of 17 (level required for pulling the lever is 15), and the thing injures her. I try it a few more times, and the ****ing thing kills her.

Ok...so I thought I'd do it the other way. I intimidated the dude into doing it for me, and he says he will...except he doesn't! He just doesn't! He wanders around the goddamn library! Thinking it may just be because the game is stupid and I have to wait a while in order for him to go pull the lever, I keep the game unpaused and go watch a few episodes of Seinfeld. I come back...he hasn't done **** and the mandrake root is still there, unchanged.

THEN I hacked in the godblasted sabotaged potion in with a console command, but the ******* that needs it won't recognize it because I need to go through the alchemy process first!

This is so typical. A similar thing happened to me when playing Baldur's Gate for the first time many years ago. I got to the end of the game and a single, then-unknown bug forced me to spend weeks trying to hack my way past it. I do not want to have to do that with this game. Please, for the love of everything...help me!

Did you remove the real mandrake root before pulling the lever, leaving only the modified root there?

Yep. As soon as I do that I get a journal entry that says exactly that. Then I send my alchemist to pull the lever.

Which lever am I supposed to pull? The FAQs I've read have been remarkably vague when it comes to this issue. I have tried both obviously, but one doesn't change it and the other kills my alchemist.

It's the south lever. When you first get there, I believe it is not operational. You have to wait until the alchemist comes back. You should hear weird noises and see green lights around the tables.

Are you patched up? I haven't had any problems dealing with this area, so your difficulty is unfamiliar to me.
When your back is against the wall... the other guy is in a whole lotta trouble.

Yeah, I'm patched up.

Well, I loaded the closest save I had that was before I had entered the area with the mandrake root, went through the entire process of finding the sword, giving it to the ghost, etc. and eventually managed to create the crap lousy potion. Problem is, I wasn't expecting it to work so I didn't necessarily mind when two of my characters were killed in the sword retrieval process. At least I know that I can fix it now.

It was kinda weird, though. The two people that died were my fighter that had points in intimidation, and the evoker that had points in alchemy. When I got to the alchemist's room, I couldn't intimidate the guy into doing it for me and I couldn't pull the lever myself. Eventually he just went and pulled it anyway.

Eh...I wish this kind of thing wouldn't happen to me so often. It's annoying! If only these games weren't so damn...fun.

Anyway, thanks for trying to help guys.

I do not believe this. I did the entire thing over again, this time making sure to keep my party alive. IT'S DOING IT AGAIN. What the **** have I done to deserve this?

EDIT: Success! Before I met the alchemist for the first time, I sent a guy in and put the poison as well as the root into the tables. A moment later, the bastard flips the switch and I end up with everything I need! I am the best!!! fwababababababab!!!!
