I would have asked this question in "Evil characters in good parties if it hadn't been closed. I was very clumsy and dropped my helm of opposite alignment at the beginning of the game, having acquired it in BG1. This was with the intention of giving it to Viconia. I am now at Rep 16 and she does not seem at all distressed by it. Clearly she could walk out on me, it all depends on the programming. What I would like to know is: Will the helm stop her walking out on me, or do I have to rely on my high charisma? Indeed, without using the fixpack, will I be able to keep her in my party if my reputation goes higher?

The helm only changes your alignment. It has no effect on reputation, so it wouldn't stop a good character from walking out if your rep falls too low, or an evil character walking out if your rep gets too high. You'd need a mod to change that.
