Where can you get one of the water breathing amulets. I've tried the search option and nothing came up.

I think they're just leveled loot. You'll find one once you're at a high enough level in a chest or on a person. You can easily make your own amulet at an enchanting shrine at the Arcane University.

But, if you're playing Oblivion on PC, you can add it using the console. Press the ~ key and then type in

player.additem 00098440 1

I think that that is the right amulet, but I'm not sure.

Yep, I confirmed in the construction set they only occur as leveled loot so it might take a while to find one at random. I agree wtih lifeishell91 the only sure fire way to get one is to make it yourself. Unlike most enchantments that are stronger if you use a bigger soul gem when enchanting, waterbreathing doesn't have any variable effects so if you go that route use a petty soul gem. It will work the same and you can save your bigger soul gems for better use.
Can the answer to this question be "No"?

I think I got one for killing the Rumare slaughterfish and collecting their scales. Quest rewards are not levelled as far as I know.
