I can't believe they haven't fixed this crap yet. It never happened to me before, so I never bothered to read the solution.
What am I supposed to do if one of my companions refuses to follow even when AI is on and puppet mode is off? Note: the other party members will follow him, but I don't want to have to lead with him all the time.

I'm not sure if this applies here, but the AI follows poorly in driving mode (not the camera setting, I mean when you're using the cursors or mouse-click-drag function to 'drive' around). Best to click on the ground every now and then, especially when you're passing through doors or past corners. At those moments you kinda simplify the following-action the AI has to do, so it will be less likely to get stuck.

I can't remember the exact wording for this, but select the character you want to lead, and then right click on the ground and select "follow me" from the menu that pops up. The right-click commands trump the AI options.

Note that you can issue commands using the character you're currently actively controlling. These commands can force everyone else to "Stand Ground", "Follow", "Guard" etc, and this applies to all your companions in the party, minus the one you're controlling when issuing the command.
I believe swcarter has it right there... but you can also access the option by right clicking the portrait or the character you're controlling. You can also give a specified set of instructions specifically to the errant character by clicking on him or his portrait. This will issue the command only to him and not affect other members of the party.
