Multiple Items?

I am running another vanilla game and have noticed a strange (but nice) issue. On more than a few occasions I have found multiple (duplicate) magic items in spots where there should be only one. I found 2 Helms of Balduran, 2 Rods of Terror, 1 Mace of Disruption (but when I had Cromwell upgrade the weapon it produced 2 upgraded maces!). While I like this phenomenon I know something is amiss. Any idea what causes this I really don't cherish the thought of double jack-in-the-box Kangaxx :speech:
