What is the best place to hunt in hell difficulty for items

"What monsters drop is random, but boss monsters and monsters with names have a higher chance of dropping higher quality items since item dropping is regulated by what "drop classes" a monster has. You need a lot of luck to get really good item drops though. That's why people do Meph-runs, Pindle-runs and Baal-runs, because these monsters have a relatively high chance of dropping good items but are relatively easy to find. Personally, I think it's boring to do 100's of runs for the same monster, so when I MF, I usually clean out areas that have a large amount of high level monsters, such as Chaos Sanctuary, The Pit or River of Flame."(...thank you C Elegans)
"Doing a Pindle run is to play as quick as possible only to kill the monster named Pindleskin, and pick up what he drops. Pindle runs are quite popular because he is so easy to reach.
You find Pindleskin in act 5, through the portal that Anya sets up for you. He stands right in the hallway in the first area. If you do not take the waypoint that is on the way to Nhilatakh(sp?) Anya's portal will remain in place, and give you easy access to a very good source of items.
The Pit is a cave in Tamoe Highland, act 1. However, It's only a good source of items on hell difficulty. If your playing on nightmare I think killing Mephisto is a better source.

Piff, Joe has kindly already posted my personal favorite hunting grounds, but it also depends on what items you hunt for and what class you are. Cow level is good for jewels and charms, killing the High Council is good for runes and charms, Meph is good for many high level set and uniques as is Baal of course, but Baal takes a little longer time to reach. Pindle is easily accessable and he is one of the few who can drop almost every item in the game, but he doesn't do it very often compared to Baal or Meph. Chaos Santuary is perfect for rune and item hunting, and you can as well take the seal bosses and Diablo while you're at it, but it's tedious for a melee char with all the Iron Maidens the Oblivion Knights cast. For a Hammerdin or Bowazon, it's piece of cake though.
The Pit is good because it's easy, but it's also quite boring to me honest. The Pit is not at the River of Flame, but in Tamoe Highlands. I prefer River of Flame though.

Is mephisto the only place you can find rainbow facets? It is the only place I have found them but am unsure since I have not found many. Mephisto/Andariel are my favorites with my enigma hammerdin as they die in 6 hits. (Yes, on hell)

I think you can find facets in every Hell area and some late NM areas. I'm not sure though, I'll have to check it. Btw, except Annihilus and the Hellfire Torch, no item is limited to a specific monster.

What is the hellfire torch??
After years of gameplay and being one of the highest ladder characters I have never heard of nor encountered this 'item'? Is it something to do with patch 1.11b? As I havent played in a long time and only played 1.11b for about 3 days and then I remembered why I quit online play.
