Quest Icons dissapearing

Quest Icons dissapearing

I have been playing single player and have the same issue on mercenary and veteren levels. In act 2 I collect all of the quests in town and when I save and exit all of the Secondary quest Icons dissapear accept the first 2. It will allow me to continue with some of the quest if I talk to the NPC but not all. How can I fix this or avoid it in the future? It is driveing me mad.
