This achievement is gained by finding 3 codex entries in each act. However, there are actually 4 entries possible per act.

Except: the second codex entry for the Band of Three is bugged.

Whomever's doing the Walkthru on Gamebanshee is likely to miss the bugged one. Especially on PC.
