This time I think it is truly a bug. I'm infiltrating the Cult of the Unseeing Eye for the first time (I simply destroyed it when I played for the first time, and now I see how much I have missed). But there appears to be a bug.

Fine, I got this piece of rod from the Avatar. I assembled it with the other one and I used it on the Unseeing Eye (I checked the walkthrough after the bug, and it says that it was the right thing to do). Then I returned to the Avatar and the Guardians, and... nothing. I even checked a walkthrough and it says that the Guardians should enter the Temple and pass away with their Old God. But nothing happens. Just as if I had gone there for the first time.

Don't use the rod/wand on the unseeing eye, or anything, it disappears after one use. It's a stupid bug since the quest tells you to do that to kill the eye. Just kill the eye the old fashioned way.
Remember, being Evil in Baldur's Gate 2 is VERY fun...

It shouldn't disappear, it should be transferred to your inventory. However if your inventory is full, or the rod fills it and you subsequently pick up something else the game dumps any excess items on the ground, including essential quest items. This bug has never been fixed, either by official or unofficial patches, you just have to be wary of it and constantly check you inventory, always making sure you have spare capacity. If you find it has disappeared all you can do is either retrace your steps, or use ShadowKeeper to replace it.

I double-checked my inventory and I had exactly one spare slot (I'm always having trouble with them when I have to go much without seeing a merchant - Throne of Bhaal has fixed it with Potion Cases, Ammo Belt and another Bag of Holding). I did this multiple times, so I used Shadowkeeper (and yes, there are two Rift Devices - one wand, that is usable, and the other one that I should have got, a simple unusable item).
