The ancient rusted key is in Rm 32 in the Main Halls if you look at the map of the Main Halls. I had the same problem trying to find the key when I started the game. Searched high and low for it and still could not find it. If you have the key on you and go near the ceremonial column, you will be teleported to a secret room below in the Deep Halls. Alternatively the key will open a door which will lead you to a room with tombs and u can go down that way. There is no other way of reaching this section of the Deep Halls without the key. You get a few goodies but not really necessary as u get much better ones later on. Sometimes when you fight the enemies and one of them run away c/o morale failure, this may be the one who has got the key on him. I can't remember if that was the case for me, but that could explain the non-appearance

The path is blocked with a secret wall - search the wall to the south to find it and open it.

Also chesk area #43 on the map.
