Hi,
I've played through SoA a couple of times and have decided to buy ToB and start a completely new game. Can anyone enlighten me on significant changes during the normal SoA campaign that might affect my spell picks, proficiencies etc.? Also, do I need the old patches - to get the bonus merchanct for instance - or are this included in ToB?
Any advice is greatly appreciated.

Someone says that ToB holds (only) one of the bonus merchants.
But if you have the patch which holds both of them, installing it can do no harm.
You will obviously need the latest ToB patch, you should not need the SoA patch anymore.

Can you be more specific?
Are you planning to start a new *SoA* game or a new *ToB* game?
These are two very different things...

For one thing, if you start from SoA with ToB added, the xp cap is history, or at least, effectivley so. You Won't stop gaining levels the way you did without ToB unless you had the Cap Remover patch
I also adds Watcher's Keep, which you can access in either SoA or ToB segment

There'll be no more alignment conflicts, thus you can powergame.

Also, several items are different, such as the Red Dragon Scale armour is now magical, so you can't wear that Ring of Protection +2 anymore with it...
Also, the Cloak of Reflection was changed to the Cloak of Deflection (the cloak you get in Saughin City, used to be one of the most powerful items in the game).

Thanks all, I'll be starting my new SoA game tonight or tomorrow. I've downloaded the latest ToB pacth but what would you suggest I install as well, as I'm sure there are some enhancements out there I don't know about?

Many people find it better to try the unmodded version of a game at least once before installing any mods, and that's my opinion as well.
If you're bored with SoA itself, you may want to install, say, the Tactics mod from Wes Weimer, but this is another matter entirely.
So I would say, try at least one unmodded ToB game to start with.
(Maybe creating a new character directly in ToB, why not: is one of the options available, and a very useful one if you're curious about high level spells and abilities).

Also the best new feature of ToB, the "Tab-key"
press it and all chests / doors / loot will light up
