Hearth of [Charname]

I bought the sword in the begining, thats called Hearth of [Charname] (in my case Leto)

Suddenly in chapter 4 it changed it's name to one of my other character.

How could this happen?

Also is there something special about this sword?

It changes name according to whoever is leading your party

I sold it in Chapter 2 IIRC, it was nothing special, just one of the better swords early on.

it was in the bag of holding.

My sword changed name when my leading character died

As soon as she was resurrected the name changed back.
