I'm sure I've been over at least 90 quests (both good and evil) but can Interplay, Blackisle and all the others think of fresh and unique ideas for quests or have they all been done.

I think that one of my favorite quests was the Jaheira curse quest. That one was great role-playing, running all over the city as Jaheria coughs and chokes her way to vengeance.
As far as more ideas for quests, how about:
-defending a building; say, the order of the radiant heart, from attack for a few days. You could wander off... and miss the sneak attack of seven assassins.
-a specific monk quest. Maybe involving the guarding of a sacred tome/item
-a mission to trade with another city, acting as ambassadors. This would be one instance where reputation could be crucial
