Trouble with hankerchief-blood
I´ve done everything you are supposed to to receive a portion of blood from Kessai-Serris, she has talked to her father but still I have no hankerchief with her blood on. When I initiate dialogue with her again I can´t talk to her about it either. It seems I´m all stuck and have to replay the whole game until then...

Coaxmetal bug?
I also have a big problem with coaxmetal, he has a dialogue option where he can evaluate your weapons, but only brings up a shop. You are supposed to be able to change the mordrom cube there into a weapon also, but it seems impossible to get a dialogue option and talk about the mordrom cube.

Brothel Issue
Firstly, have you installed the patch? If there is a bug, installing the patch now will not fix it, but may prevent future problems from appearing.
The official patch is here.
There is also a great fan-made patch.
Secondly, I'm unsure of the exact conversations, but make sure that you
a) Have a handkerchief on you.
b) Have visited the Private Sensoriums and talked to Ravel
c) Have exploited every single one of Kessai's dialogue options (recheck).
d) Tried talking to every single member in the Brothel again about *everything* in case a trigger got lost someplace.
If all else fails, have you got any previous savegames to fall back to, preferably shortly before the Brothel?

The store is supposed to open up when you ask him to evaluate your weapons. To get the Blade of the Immortal, you have to carry on the conversation after the shop a bit further:
1) Ask who it makes the weapons for.
2) Continue that section of the dialogue until you can say "So you're saying immortality is just a different kind of death?"
3) Then ask him he can make a weapon to kill an immortal
4) Go forward until this option opens up: "what if enemy strikes from the shadows". He will then eventually give you an option to create the Blade.

About coaxmetal, whatever options I choose I cannot get the option to forge a weapon from the modron cube, but I got a waepon which could kill immortals though.

I´ve checked your list and a,b,c,d seems to be fullfilled all of them, and I´ve spent hours trying to get new dialouge options but no. But thanks a lot for trying to help, thats noble!

Can't remember precisely, but after confronting Kessai-Serris, you also need to talk to Kimasxi, and then to K-S again, where you should get the option to ask for a piece of her.
Modron Cube Spoiler (highlight)
The Modron Cube opens a portal to the modron maze. Talk to one of the modrons at the brothel about it, then extend the left wing extend the right wing, then rotate the right arm.

You can't give the Modron Cube to Coaxmetal for a weapon until you come back to Sigil after having left Sigil to find Ravel.
For the Ravel quest, make sure you have talked to Lothar and done the quest for him, so he will answer your questions. Many people miss this one if they don't have Morte when they first enter the Lower Ward. Lothar is the one who actually tells you that you need to find Ravel in the first place.
