The only way to regenerate in BGI (that I know of) is to get a 20 (or higher) CON. This is only possible with a Dwarf, or by using the "Draw Upon Holy Might" spell/ability.

The regeneration opcode in BG is rather annoying in that it only works with a limited timing mode. Thus it is not possible to make a ring of regeneration that works while worn. A spell with a limited duration would be possible, though.

could you make an item that regenerates for only a short amount of time, but triggers itself to restart right when it finishes, effectively being a permanent regenerating item?

If not, make a ring or amulet that sets your con to 20. It'll have some side effects (higher HP, better poison resistance IIRC) but it will give regen.

You could make a ring with a limited regeneration ability but it would be incapable of restarting itself. Once its duration runs out the player would have to re-equip it.
