Hop on over to the "Debt to Sarevok" thread and you'll see what Polaris' biggest dissapointment is (the Solar's question of course!).
My biggest dissapointment is that is was too linear.

To few NPCs. I would have wanted them to include a few more NPCs available right from start in SoA. More NPCs means more replay value. The least they could have done was to reinstate Yoshimo.

I don't think there is an 'improved' Yaga Shura, but he is *supposed* to be a neutered shadow of his former self by the time you fight him. Has anyone tried fighting him before neutering him?

My biggest gripe is that it seems like you always end up fighting a whole lot of the same enemies at once, and they all "quaff a potion"....I don't know, it just irks me that everytime I go into combat everyone I am fighting is drinking. It just seems to me that large amounts af enemies were put there in a cheap effort to make the game difficult. Oh well, just my ramblings on the subject.....

What Aegis said pretty much sums it up for me. Add to it that some of the endings sucked except for a handful (Keldorn, Edwin come to mind for the "non-sucky" endings).

Melissan. I had hoped for something grander and more epic than just another (cheating) mortal psycho with delusions of divinity as the end boss. Besides, she was transparent and shallow from the get-go, not to mention that her story (regarding her place in Bhaal's original plans) makes absolutely no sense to me.
Second prize goes to the collective inconsistencies. Did Bhaal make his home in the Nine Hells or Abyss (neither, actually)? Was the PC's mother Gorion's friend or a cleric of Bhaal? Was Sarevok adopted or not? How old are the PC and Imoen?
While I'm at it, the inconsistency of your supposed versus your actual power also ticks me off, combined with the totally ridiculous number of high-level enemies, all complete with their own +3 armor and weapons. Such powerful people and items are supposed to be rare, but someone must be mass-producing them somewhere.
Other gripes are the lack of seperate 10th/8th level spell pages for wizards and priests, and Bio's apparent continuing inability to fix spelling errors and inconsistencies.

Hmm...About the levels.
Some seem to think that the PC becomes too powerful, but the avatars of the god actually were very powerful indeed - and thats just the avatars.
A greater power - like Bhaal was - had 10 avatars at a time.
I can't remember them, but as an example, lets take Helms avatar:
A human of giant proportions: 12 feet tall. Cound fly.
level 20 cleric AND level 35 fighter.
Besides that he had a TON of very powerful abilities...
To me it would be a joke if a level 20 or so character could become a god.
Now, if the game isn't challeging enough for a 40th level character, that's totally different...

The fact that the game is linear is so annoying! Another thing which really sucked was the last "Fight". Melissan is a whimp and the story (which Nightfire summed up nicely)that ties her to bhaal is just stupid...
I get the feeling of ToB was made to quickly to my liking, and a grand saga should have a GRAND ENDING wich ToB isn't... well that's just what i think
