We initially only knew of the title as Project Beast and only learned about it through unofficial sources that pinned it as a Souls-like PS4-exclusive from From Software, but now the elusive action-RPG has finally been confirmed to exist and given the official title of Bloodborne thanks to Sony's E3 conference. Unfortunately, the only thing shown was a CGI trailer:

Update: The PlayStation Blog mentions that the game will be shown in more detail later this week:
