Those of who you've played CoH or CoV know that missions are delivered to you by either visiting a contact or reading the newspaper. Originally, after a few random missions, you would get the opportunity to rob a bank. Basically, Mayhem Missions are bank-robbing adventures on steroids. Instead of running in, beating up a few police officers and pounding on a bank vault door, the action starts as soon as you set foot in the zone. The area will be crawling with police, and Longbow squads will regularly pop in and try to put a stop to you. Since a Mayhem Mission is timed, you don't want to get bogged down searching for baddies to fight, but a successful battle will add more time on the clock, as will destroying groups of parked vehicles, and fire hydrants and pay phones.
During the course of the job, an enemy may drop a key that leads to an optional side mission. Besides opening up an opportunity for more loot and experience, completing the side missions grants clock time. Also, in this mission mode, dying does not return you to a hospital. If your group is eliminated, you'll be thrown in jail instead, and you must fight your way out of the precinct (which is chock-full of high-ranking police). A fellow villain might escape alongside you, and this randomly selected, AI-controlled villain will join up with you for a while, in return for setting him free.
